Why High-Functioning Anxiety Often Goes Unnoticed

Sometimes anxiety does not look the way people expect it to.
It may not look like panic attacks or visibly falling apart.
Sometimes, anxiety looks like being highly productive, organized, dependable, and calm on the outside. But internally, many women feel mentally overwhelmed, emotionally tense, and unable to fully relax.
This is often described as “high-functioning anxiety” which is experiencing significant anxiety while continuing to function at a high level externally (Mayo Clinic Health System, 2023).

When Anxiety Is Hidden Behind Achievement

Many women with high-functioning anxiety appear composed and successful externally while privately struggling with:
Constant overthinking
Fear of making mistakes
Pressure to stay productive
Difficulty slowing down
Persistent self-criticism
Because they continue functioning, their anxiety often goes unnoticed by others, and sometimes even by themselves.
People with high-functioning anxiety may appear highly accomplished externally while internally struggling with stress, perfectionism, and fear of failure (Mayo Clinic Health System, 2023; Maenpaa, 2022).
Common Signs of High-Functioning Anxiety

High-functioning anxiety can show up as:
✅ Overthinking and overpreparing
✅ Difficulty relaxing during downtime
✅ Constant mental planning
✅ Replaying conversations afterward
✅ Perfectionism and people-pleasing
✅ Appearing calm externally while feeling anxious internally
Many women also experience physical symptoms such as muscle tension, headaches, digestive discomfort, sleep difficulties, racing thoughts, and feeling constantly “on edge” (Mayo Clinic Health System, 2023; Maenpaa, 2022).
Why It Often Goes Unnoticed

Many behaviours connected to high-functioning anxiety are socially rewarded.
For example:
Being highly productive
Taking on extra responsibilities
Staying constantly available
Appearing composed under pressure
Externally, this may look like success.
Internally, it is often driven by fear, perfectionism, chronic worry, or fear of failure.
Many individuals with high-functioning anxiety become skilled at masking emotional distress while continuing to perform well professionally and socially (Mayo Clinic Health System, 2023; Maenpaa, 2022).
Some also struggle with bottling up emotions, overfunctioning, and difficulty asking for support (Maenpaa, 2022).
The Added Pressure Many Women Carry

For many Black and racialized women, anxiety may also exist alongside workplace inequities, racialized stress, caregiving responsibilities, and pressure to remain resilient and composed.
There may be pressure to:
🟢Always appear capable
🟢Work twice as hard
🟢Avoid making mistakes
🟢Remain emotionally controlled
Over time, constantly carrying these pressures can become emotionally exhausting.
These experiences are real and deserving of support.
When Functioning Well Does Not Mean Feeling Well

One challenge with high-functioning anxiety is that many women minimize their distress because they are still “getting things done.”
They may think:
💭 “I’m functioning, so maybe it’s not that serious.”
💭 “I should be able to handle this.”
But functioning externally does not mean your nervous system is not overwhelmed internally.
Over time, chronic anxiety and emotional hypervigilance can contribute to burnout, exhaustion, perfectionism, sleep difficulties, and chronic stress responses (Mayo Clinic Health System, 2023; Maenpaa, 2022).
Learning To Support Yourself Differently

Healing from chronic anxiety is not about becoming less capable.
It is about learning how to function without constantly operating from fear, pressure, or survival mode.
This may begin with:
✔ Becoming more aware of perfectionism and over functioning
✔ Learning healthier emotional boundaries
✔ Allowing yourself to slow down without guilt
✔ Recognizing that support is allowed before complete burnout
Counselling approaches such as CBT, mindfulness, grounding techniques, and emotional awareness can help individuals better manage high-functioning anxiety and chronic stress patterns (Mayo Clinic Health System, 2023; Maenpaa, 2022).
